Guiding Light is a palliative program by The Carpenter Health Network that offers specialized palliative services for patients facing serious or advanced illness. We provide symptom management and care coordination alongside your care team.
Who is this for?
Guiding Light may be helpful if you or a loved one is living with:
- Heart disease or heart failure
- Lung disease (COPD, breathing problems)
- Cancer
- Dementia or memory loss
- Parkinson's Disease
- Kidney disease
- Liver disease
- Stroke or neurological conditions
- Other serious chronic illnesess
How we support patients & families
Symptom & pain management related to your diagnosis
Regular in-home or telehealth visits by a Palliative Nurse Practitioner
Additional in-home nursing support by Medical Assistants
Frequent Nurse or Care Navigator check-ins between visits
Coordination of care with primary doctors & specialists
Education & support for patients and caregivers
Advanced care planning conversations as appropriate
Before Guiding Light, I felt overwhelmed trying to manage medications, appointments and decisions. Their team coordinated with our doctors and helped us understand our options. They gave us peace of mind and reminded us we weren't alone.
