Advanced care planning conversations as appropriate

  • Before Guiding Light, I felt overwhelmed trying to manage medications, appointments and decisions. Their team coordinated with our doctors and helped us understand our options. They gave us peace of mind and reminded us we weren't alone.

  • We didn't realize this kind of support was available while my dad was getting treatment. Guiding Light helped control his pain, explained everything in a way we could understand and made us feel like we finally had someone walking this journey with us. The hospital visits slowed down, and our stress did too.

What's our mission?

  • Help patients stay comfortable & safe at home
  • Reduce avoidable ER visits & hospitalizations
  • Support caregivers & families
  • Offer a seamless transition to hospice when appropriate
