STAT Home Health’s team of compassionate, experienced healthcare professionals bring optimism and a “can do” attitude to every visit. STAT services focus on reducing complications following injury, surgery or illness; and increasing patients’ recovery rate. We also provide guidance and education to patients and their families — so that no matter what challenges you’re facing, you can move forward with confidence and certainty.

By delivering specialized care directly to a patient’s home, we allow them to receive the services they need in the most comfortable setting possible.

STAT Home Health services fall into four categories:

1. Assessments: A thorough health and/or home assessment can help ensure that a patient receives the individualized care that they deserve. The STAT team can provide in-home physicals, low vision, and health assessments to determine whether an individual would benefit from home health care, and if so, which services would be most appropriate. We can also conduct a home environment assessment to identify potential challenges present in an individual’s home. Once an assessment has been taken, a STAT caregiver will work with your or your loved one to ensure that you can easily manage day-to-day tasks within your own home.

2. Specialized programs: STAT Home Health offers detailed programs to help patients in areas such as low vision management (for patients facing conditions such as glaucoma, inoperable cataracts, multiple sclerosis, and retinal tears); fall prevention (for patients looking to improve functional strength, balance, and gait, as well modify the layout of their home to meet their needs); medication management (to help patients identify medication side effects, learn to prepare medications, and create a medication routine); diabetes management (to help educate patients on how to manage diabetes, plan meals, and identify risks); and more.

3. Clinical care: In-home clinical care allows patients to recover and regain strength — without having to worry about leaving the comfort of their own home. STAT Home Health provides a number of clinical services to help patients recover from injury, surgery, or illness, thus helping them to better maintain good health and improve their quality of life. Clinical services include rehabilitative therapy, wound care, IV therapy, injections, anticoagulation therapy management, assistance with bathing/dressing/toileting, and more.

4. Education: Education is one of the best preventative medicines. The STAT Home Health team is trained to educate patients and their families in areas including special diets; diabetes; disease processes, reportable signs, and symptoms; ambulation and/or activities of daily living; self-administration of medications; and compliance with treatment plans.
