Hospice continuous care, often called hospice crisis care, is an end-of-life option that provides the patient and family around-the-clock home care at bedside mdash; ensuring you have a nurse on call for immediate assistance during times of crisis. As part of the four levels of hospice care, our 24-hour care typically consists of skilled nursing services and support for both the patient and his or her loved ones.

Our on-call hospice continuous care team can provide medical treatment, nursing care, counseling, and more — oftentimes allowing patients to avoid a stressful trip to the emergency room. At The Carpenter Health Network, hospice crisis care is available 24/7 for those facing an emergency such as uncontrolled agitation, uncontrolled respiratory distress, uncontrolled pain, or a family crisis.

Our hospice crisis care team is permitted to provide around-the-clock-care for up to 24 hours, with patient appropriateness reassessed every 24 hours, per CMS guidelines. We understand through experience that times of crisis can be stressful for both patients and their families, and our goal is to provide comfort and calm during these moments.
